This 30 minute Zoom session includes the following: • Brief Holistic (mental, physical, emotional and spiritual) Health History and Current Needs Review • Holistic Health Education • Suggestions for Optimum Health & Well-Being based on health history, current needs and holistic assessment. Please note: An intake form must be completed for this session. See the Documents page under the “More” tab to download the appropriate intake form. Your completed form must be received by Practitioner prior to this session.
